Wisconsin State Statutes, Section 174 makes it mandatory for pets to be licensed.
Due by March 31, 2012 – Please AVOID licensing at City Hall during DECEMBER & JANUARY.
The City of Oak Creek requires a license for dogs (limit two) and cats over five months of age. You can license your pet online, by mail or at the Oak Creek Treasurer’s Office (see options below). Residents must provide a current certificate of rabies vaccination (showing the tag number and the expiration date) at the time of licensing. The license year commences January 1st and ends December 31st. Beginning April 1st, a late fee will be assessed except for:
New Residents and New Pet Owners:
- A new resident who owns a dog or cat over five months of age, and who licenses such pet within 30 days of residency or;
- A resident who obtains a dog or cat on or after April 1st, and who licenses such pet within 30 days or,
- A resident who owns a dog or cat that is not five months of age on or before March 31st, and who licenses such pet within 30 days from the date in which it reaches five months of age.
You should receive a 2012 renewal postcard from Milwaukee Area Domestic Animal Control Commission (MADACC) in December or January, unless you licensed your pet after October 31, 2011.
Whether or not you receive a renewal postcard, it is your responsibility to license your pet within the required time period.

ONLINE, with a credit card at www.madacc.com. Add $1 to License fee for this service.
BY MAIL, to Milwaukee Area Domestic Animal Control Commission, 3839 W. Burnham St, West Milwaukee, WI 53215. Include the postcard (for renewals), current certificate of rabies vaccination (will be returned with license) and check made payable to MADACC. Call (414) 649-8640 for questions. Add $1 to License fee when mailing directly to MADACC.
AT CITY HALL, mail or bring the postcard (for renewals) and current certificate of rabies vaccination (will be returned with license) to 8640 S Howell Avenue, PO Box 27, Oak Creek, WI 53154-0027 with checks made payable to Treasurer or to City of Oak Creek (CHECKS MUST BE SEPARATE from tax payments).
If your license is considered late you must include the applicable late fee with your payment.
