Administrative Fee for Children: Effective January 1, 2011, the administrative fee will be $10.00 per child⁄per visit for residents and $15 per child/per visit for non-residents.

 

If you are covered by BadgerCare+ or Medicaid, please present a current Forward Card to have the fee waived. Children under 18 years old must be accompanied by a parent or guardian. A copy of your child’s Immunization record is required prior to clinic visit.

 

Fees for Adults:
Vaccine availability based on Department of Health and Family Services, Wisconsin Immunization Program, eligibility guidelines. Administrative fee included in cost of Pneumonia vaccine.

Immunizations
Oak Creek Health Department

Below are some of the current vaccines offered by the Oak Creek Health Department.

DTaP - Diphtheria, Tetanus, acellular Pertussis (Whooping Cough)
Offered to children age 2 months through 6 years. Series of 5 injections: the first 3 are given 2 months apart, the 4th is given 8-12 months later, & the 5th BEFORE school entrance. (AFTER age 4).

DTaP, IPV & Hepatitis B combination
Offered at 2, 4, and 6 months of age or as indicated for children behind schedule.

DTaP, Hib, IPV combination
Offered at 2, 4 and 6 months of age or as indicated for children behind schedule.

HEPATITIS A (HepA)
Offered to children ages 1 - 18 years. 1st dose recommended between ages 12-23 months. Series of 2 injections with the second dose administered six months after the first dose.

HEPATITIS B (HepB)
Offered to children 2 months of age and older. Series of 3 injections: the first 2 are given 4-8 weeks apart and the 3rd dose 5-12 months later.

Hib - Haemophilus influenzae b
Offered to children age 2 months through 4 years. Series of 3 or 4 injections.

HPV - Human Papillomavirus
Offered to children 11 through 18 years of age. Series of 3 injections. The second and third doses are administered 2 and 6 months (respectively) after the first dose.

IPV - Polio (Injectable)
Offered to children age 2 months through 18 years. Series of 4 injections: the first 3 are given 2 months apart and the 4th after age 4.

 

Meningococcal Conjugate
Offered to children ages 11 to 12 with a booster at age 16.

MMR - Measles, Mumps and Rubella
Offered to children ages 12 months and older. Series of 2 injections; based on child’s age.

Pneumococcal Conjugate
Offered to children age 2 months through 4 years. Series of up to 4 injections, based on child’s age.

Pneumococcal Polysaccharide
Offered to adults; second dose recommended if first dose received prior to age 65.

Rotavirus
Series of 3 oral doses. Offered to infants 2 thru 7 months of age. First dose must be prior to 14 weeks 6 days of age.

Seasonal Influenza
Offered to anyone 6 months of age and older.

TD – Tetanus and Diphtheria
Offered to anyone 7 years of age and older.

TDaP – Tetanus, Diphtheria, acellular Pertussis
Offered to anyone ages 10 years and older or children 7-10 years of age with an incomplete DTaP series.

VARICELLA - Chicken pox
Offered to children ages 12 months and older. Series of 2 injections; based on child’s age.

Tuberculin (TB) Mantoux Skin Tests

Tuberculin tests are given EVERY Monday - 8:30 to 10:30 a.m. or by appointment at a cost of $15.00 per test.

Test results must be read within 48-72 hours.
