Recommendation:  That the Common Council approve the recommendation of the Board of Health on October 4, 2007, to enter into an Agreement with the University of Wisconsin College of Nursing to assist the Oak Creek Health Department with the development of the Community Health Plan.

 

Background:  The Oak Creek Health Department is mandated to perform a Community Needs Assessment every five years and develop a five-year strategic health plan.  The last Community Needs Assessment was performed in 2003 and the Health Plan developed later that year.

 

In 2006, Aurora Health Care conducted a community health survey (at no cost to the Health Department) and shared the results with the Oak Creek Health Department. 

 

There was a meeting on September 28, 2007, between the Health Department and UW-Milwaukee to discuss the responsibilities of each party as well as the cost for services.  An Agreement was drafted by Elizabeth Fayram, PhD, and submitted to the Health Department on October 4, 2007; it was presented to the Board of Health on that same date. 

 

A motion was made by Dr. D’Amico and seconded by S. Schneider to allow the Health Officer to move forward with the aforementioned Agreement.  It passed unanimously.    

 

Fiscal Impact:  To pay $1,200 in 2007 by utilizing funds from the Prevention Grant.  Provided the Health Department is awarded the Grant in 2008, to pay the remaining cost of $2,600 out of those funds.  In the event the Health Department does not receive the anticipated Grant, this cost has been included in the 2008 Health Department Budget.
