COMMON COUNCIL PROCLAMATION

TO ROBERTA ZAPP

FOR DEDICATED SERVICE TO THE CITY OF OAK CREEK

AS A MEMBER OF THE

BOARD OF HEALTH

WHEREAS, Roberta Zapp was appointed to the Board of Health in May, 2003; and

WHEREAS, Roberta Zapp has resigned from the Board of Health; and

WHEREAS, Roberta Zapp contributed to policy decisions regarding the Health Department, such as grants, bioterrorism, beach water testing, and other issues; and

WHEREAS, Roberta Zapp has served the City in a thorough, conscientious and professional manner.

NOW, THEREFORE, be it resolved by the Mayor and Common Council of the City of Oak Creek, for and on behalf of the citizens of the City of Oak Creek, that sincere gratitude and appreciation be extended to Roberta Zapp for her dedicated service to the City of Oak Creek as a member of the Board of Health.

BE IT FURTHER RESOLVED that this resolution be spread upon the minutes of this meeting and that the City Clerk is hereby directed to transmit a suitable copy thereof to Roberta Zapp.
