Recommendation:  That the Common Council approve the Community Health Improvement Plan, 2003 for the City of Oak Creek.

 

Background:  Federal grants require that communities with health departments periodically assess the health status of the community and develop a health improvement plan.  The plan must be formulated with input from citizens within the community.  The Board of Health sponsored this activity earlier in the year with funding from the health department’s prevention grant.  The Board of Health approved the plan at its June 16th meeting.  The board requested the CPHA to prepare fiscal implications to implement the plan (attached).  This was completed in August; none of the proposed activities will be implemented at this time unless funding is appropriated from grants or other sources. 

 

Fiscal Impact:  None at present time.  The receipt of Prevention Block Grant Funds of $4,534 to complete and adopt the plan in 2003 and $4,529 to implement one of the programs in 2004.
