2. Roll call.
3. Review of claims:
   1. Claim submitted by Jason Cywinski, 8014 S. Wildwood Dr. in the amount of $45.00 for a slip/fall incident on 2/8/04.
   2. Claim and estimates submitted by Russell & Deborah Carpenter, 8495 S. 15^th Ave. in the estimated amounts of $349.32 and $365.62 for replacement of two tires and one rim as the result of a tire blow-out on E. Forest Hill Ave. on 7/3/04.
   3. Claim and estimate submitted by Heidi Kollenbroich, 2533 W. St. John’s Way, in the estimated amount of $433.69 for replacement of a tire and wheel as a result of a tire blow-out on E. Forest Hill Ave. on 7/4/04.
   4. Request of Robert Pergande, legal guardian for Deborah Pergande, for a waiver of fees for ambulance service on 6/12/03 in the amount of $354.00.
   5. Request of Eihab Atout for a waiver of weed-cutting charges in the amount of $594.00 on 6/20/04 for his property at 2320 W. Ryan Rd.
   6. Review Notice of Claim submitted by Judy Timber, 8930 S. Parkside Dr. dated 4/20/04 relating to an injury to Nicholas Timber at the East Middle School basketball court.
