DTaP - Diphtheria, Tetanus, acellular Pertussis (Whooping Cough)
Offered to those age 2 months through 6 years. Series of 5 injections: the first 3 are given 2 months apart, the 4th is given 8-12 months later, and the 5th BEFORE school entrance (AFTER age 4).
COMVAX – Hib and Hepatitis B
Series of 3 injections: given at 2 and 4 months of age. The last dose is given AFTER age 12 months.
HEPATITIS A (HepA)
Offered to children ages 1 - 18 years.
1st dose recommended between ages 12-23 months. Series of 2 injections with the second dose administered at least six months after the first.
HEPATITIS B (HepB)
Series of 3 injections; the first 2 given 4-8 weeks apart; the 3rd injection 5-12 months later.
Hib - Haemophilus influenzae b
Offered to children age 2 months through 4 years. Series of 3 or 4 injections:
HPV - Human Papillomavirus
Offered to female children 11-12 years of age: HPV vaccine is administered with a three dose schedule. The second and third doses are administered 2 and 6 months (respectively) after the first dose.
IPV - Polio (Injectable)
Offered to children age 2 months through 18 years. Series of 4 injections: the first 3 are given 2 months apart and the 4th AFTER age 4.

MENACTRA (MCV4) -
Meningicoccal Conjugate
Offered to children ages 11 - 18 years.
MMR - Measles, Mumps and Rubella
Series of 2 injections: the first is given AFTER 12 months old; the 2nd given BEFORE school entrance. For school age children: the 2nd dose given 1-2 months later.
PEDIARIX – DtaP, IPV and Hepatitis B
Offered at 2, 4, and 6 months of age.
PREVNAR – Pneumococcal Conjugate
Offered to children age 2 months through 4 years. Series of up to 4 injections, based on child’s age.
Td – Tetanus and Diptheria
Offered to anyone 7 years of age and older.
Tdap – Tetanus, Diptheria, acellular Pertussis
One dose offered to anyone 11 years of age and older.
VARICELLA - Chicken pox
One dose offered at age 12 months through 12 years. Age 13 to 18 years - 2 doses are given 4-8 weeks apart.
PNEUMOVAX
One dose offered to adults; second dose recommended if first dose given prior to age 65. May be given any time throughout the year. |
