ADULTS NEED SHOTS TOO!

The Oak Creek Health Department will be offering an adult immunization clinic on June 15, 2010, in the Common Council Room at City Hall, 8640 S. Howell Ave., from 7:00 AM - 4:30 PM.

The vaccines being offered are: Hepatitis A, Hepatitis B, HPV (Gardasil, to age 26; males and females) MMR (for adults born after 1957), Td, Tdap (through age 64) and Varicella. There is a $10 administration fee per visit. This is made possible through the Wisconsin Immunization Program, with funding from the American Recovery and Reinvestment Act of 2009 (ARRA). It is recommended that you go through your own health care provider for these immunizations unless you do not have insurance, have a high co-pay or deductable.

Please bring in a copy of your immunization records; contact your health care provider to get a copy if you do not have your records. Please call 768-6539 to make your appointment.
